Position Summary

The Primary Care Pharmacy Technician supports integrated clinical pharmacy services within the primary care setting. Working under the supervision of a licensed pharmacist the technician assists with medication management quality measure gap closure and care coordination. This role focuses on patient engagement data management and workflow support to enhance medication safety adherence and overall quality of care without performing medication dispensing.


Job Description

This position requires onsite training for approximately 3-6 months at our Portland Maine location. After the training period is complete this position offers a hybrid schedule.

PRIMARY DUTIES AND RESPONSIBILITIES

Employees are expected to work consistently to demonstrate the mission vision and core values of the organization.

  • Runs daily/weekly reports to identify patients with care gaps related to Medicare Stars HEDIS or value-based care measures.
  • Conducts telephonic outreach calls with patients and provide medication adherence counseling and support.
  • Educates patients on accessing refills and financial assistance programs.
  • Documents outreach and interventions in the EHR using standardized templates.
  • Tracks completion and follow-up of pharmacist-recommended interventions.
  • Reviews electronic health record (EHR) medication lists for accuracy duplication and potential adherence issues.
  • Conducts pre-visit medication history interviews with patients via phone portal or in-person.
  • Flags medication-related quality measure gaps (e.g. statin use diabetes medication adherence hypertension control) for pharmacist/provider review.
  • Assists in preparing comprehensive medication review (CMR) materials for pharmacists.
  • Serves as a liaison between providers pharmacists patients and community pharmacies.
  • Assists with coordinating follow-up appointments and ensure continuity of medication therapy.
  • Maintains logs and reports for quality improvement initiatives.
  • Assists with prior authorization paperwork refill request coordination and insurance benefit checks.
  • Supports pharmacist in managing collaborative practice agreements (CPA) documentation.
  • Participates in care team huddles and quality meetings to represent pharmacy services.

POSITION QUALIFICATIONS

Education

  • High School Diploma or equivalent.

Licensure/certification

  • Pharmacy Technician Certification (CPhT) required.
  • Active and unrestricted certification as a Pharmacy Technician (CPhT) from the Pharmacy Technician Certification Board (PTCB) or equivalent preferred.
  • CPR/BLS Certification preferred.

Experience

  • 2 years of experience working as a pharmacy technician in a clinical managed care or community pharmacy setting.
  • Experience in a clinical or non-dispensing role (ambulatory care managed care quality management).

Knowledge

  • Knowledge of Medicare Part D Stars and HEDIS quality measures.
  • Familiarity with medication adherence strategies and population health workflows.

Skills

  • Proficiency with EHR systems and Microsoft Office/Excel.
  • Strong communication organizational and customer service skills.
