What will you do

The Care Connector works within a prescribed mandate helps patients navigate through a complex health care system by connecting people to an appropriate health care provider while also managing waitlists.

This is a temporary full time (1.0) Registered Practical Nurse position.

What must you have

  • Registered Practical Nurse in good standing with the College of Nurses of Ontario.
  • Good knowledge of primary care services and other community resources as well as the roles of health care professionals.
  • Experience working with various health disciplines as well as the public interagency contact.
  • Understanding of the long-term care and health care delivery system in the province of Ontario.
  • In-depth knowledge of tools systems and databases used in patient service delivery and management.
  • Excellent interpersonal communication assessment problem-solving and decision-making skills
  • Effective time management prioritization and organizational skills with the ability to work independently and co-operatively in a busy multidisciplinary environment
  • Established ability to accurately complete required documentation reports and forms
  • A valid drivers license and access to a reliable vehicle
  • Proficiency in a Windows environment
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment all employees are required to submit proof of COVID-19 vaccination status prior to start date
