Job Summary

Since 1957 weve assisted a wide array of populations throughout Southwestern Pennsylvania focusing on those generally underserved by traditional services including senior citizens veterans individuals with physical and/or intellectual disabilities individuals on the autism spectrum and youths who have aged out of foster care.
JOB SUMMARY
The Foster Youth to Independence Program provides housing assistance on behalf of youth aged 18 to 24 who left or will leave foster care within 90 days and are homeless or at risk of becoming homeless. The initiative aims to assist these youth in achieving self-sufficiency by providing up to 36 months of rental assistance and supportive services. The FYI Coordinator supports participants in the program.
ESSENTIAL FUNCTIONS
  • Liaison between ACTION-Housing and Housing Authority of Pittsburgh staff as well as with the 412 Youth Zone.
  • Provide case management duties including school employment and referrals.
  • Meet with clients in their units in the office and out in the community once a month.
  • Help to secure housing for program participants by visiting apartments throughout the Pittsburgh area ensuring their cleanliness and safety.
  • Develop monitor and revise client goal plans.
  • Develop a discharge plan to include a two-month aftercare plan for program graduates.
  • Maintain and update all client files per regulations.
  • Document activities of casework and interaction with clients through written case notes.
  • Help move participants into their apartments. Secure donations and purchase items as needed to furnish units.
  • Teach and model independent living skills.
  • Establish/maintain relationships with:
    • ILP agencies in Allegheny County
    • HACP
    • Other service providers as needed
  • Help prepare all reports as required by funding agencies.
  • Other duties as assigned.
EDUCATION AND EXPERIENCE
  • Associates Degree in Social Services required; Bachelors in Social Work preferred.
  • Experience working with youth aging out of Foster Care System preferred.
  • Experience working with City County and County Housing Authority Section 8 voucher program preferred.

  • Maintain a physically and emotionally safe environment.
  • Develop and maintain positive relationships with clients.
  • Create an inspiring team environment through open communication.
WORK ENVIRONMENT AND PHYSICAL DEMANDS
  • General office environment.
  • Local travel required.
  • Physical strength to manipulate light to medium weight materials weighing 10 to 15 pounds on a regular basis.
