AHN Primary Care Institute is seeking a Geriatric Medicine Physician to join our growing team in Erie Pennsylvania. In this role you will deliver high-quality person-centered care in a consultative/co-management capacity to older adults with complex medical and social needs.

Highlights:

Work closely with primary care clinicians and specialty providers where you will help guide care plans that promote independence dignity and quality of life for aging patients.
Practice in a supportive collaborative environment with access to multidisciplinary team-based resources to ensure coordinated comprehensive care for every patient.
Primary outpatient with potential for limited practice in skilled nursing facilities/assisted living communities.

Qualifications:

MD or DO degree from an accredited medical school.
Board Certification in Geriatric Medicine.
Current Pennsylvania medical licensure or eligibility to obtain licensure.
Experience in geriatric care is preferred.
