The Medication Reconciliation Tech will obtain and document medication histories in the electronic medical record in an accurate and timely manner in compliance with standard procedures the Joint Commission and other regulatory standards.
Your exceptional skills
Interpret and enter Prior to Admission medication history into the EPIC electronic medical record perform thorough interviews with patients and family members conduct telephone interviews with outpatient pharmacies long-term care facilities or other locations that may provide medication history track interventions and productivity using departments process regularly and professionally communicate with patients their families and hospital staff
Your exceptional qualifications
High School diploma or GED equivalent required.
Previous pharmaceutical or related healthcare experience preferred.
Previous medication history taking experience preferred.
One of thefollowingLicensures is required:
Licensed as a Pharmacy Technician by the Indiana Professional Licensing Agency (IPLA) and Nationally Certified by PTCB or NHA
Licensed as a Pharmacy Intern by the Indiana Professional Licensing Agency (IPLA). If licensed as Pharmacy Internyou do not need to be nationally certified with PTCB or NHA
If not nationally certified upon hire will need to obtain within 2 years.
