Job Description:

• Delivers or arranges spiritual care to patients and families; this includes assessing spiritual needs responding to referrals leading groups providing care during end of-life and crisis situations and effectively documenting each encounter in the electronic medical record.
• Routinely provides weekend and overnight on-call coverage for end of life critical incidents grief and loss critical care patients religious and culturally responsive care needs etc. and to offer support to the healthcare team caring for these patients and families.
• Supports an interdisciplinary approach to spiritual care by working to ensure that the unique spiritual religious and cultural needs of patients families and employees are addressed.
• Other duties as assigned.

• Bachelors Degree from nationally and locally accredited institution
• Completion of one unit of Clinical Pastoral Education
• One year of experience as a spiritual care provider

• Masters degree in Divinity or equivalent
• Completion of four units of Clinical Pastoral Education
• Two years of experience as a spiritual care provider

• Board certification as a professional chaplain preferred
• Valid Drivers License required upon hire - MN and WI residents must obtain a valid drivers license in their state of residence within 60 days of employment in hospice setting

• Sedentary:
• Lifting weight up to 10 lbs. occasionally negligible weight frequently
