Key responsibilities

Provide all levels of care to patients in home settings
Support patients across multiple Illinois locations
Complete and submit charting and documentation by required deadlines
Manage time and schedule independently
Use tablets mobile phones and EMR software effectively

Schedule

Monday through Friday daytime hours
No weekends on-call or holidays

Qualifications
Required

At least 2 years of experience in any of the following: Home Health Care Home Health Epic Home Health (1 years)
At least 1 year of experience with any of the following: Electronic Medical Record (EMR) other EMR systems or TruChart EHR
Active Registered Nurse (RN) license in Illinois
High school diploma or GED
Valid driver s license
Willingness to travel within the assigned region

Preferred

Provide RN license number for credential verification
