This position provides compassionate hospice care and support to patients and families throughout Mechanicsburg and nearby areas. The role focuses on managing and delivering individualized care plans ensuring timely documentation and maintaining a patient-centered approach during end-of-life care.

Key responsibilities

Manage and deliver patient care plans in hospice settings
Provide support to patients and families within the assigned area
Complete documentation and charting accurately and on schedule
Manage time and schedule independently

Schedule

Saturday and Sunday 8:00 a.m. to 8:00 p.m.
One additional day (Friday or Monday) from 8:00 a.m. to 5:00 p.m.
Candidates available only for Saturday and Sunday may also be considered

Travel

Local travel within the assigned region

Qualifications
Required

Associate degree or higher in Nursing
At least 2 years of hospice experience
Active Registered Nurse (RN) license in Pennsylvania
Valid drivers license reliable transportation and current auto insurance

Preferred

RN license number provided for credential verification
