This position provides weekend home health care and patient support across Western Pennsylvania. The role involves managing and delivering patient care plans completing clinical documentation and maintaining consistent communication with patients and team members. The nurse will work independently in the field providing high-quality care with professionalism and compassion.

Key responsibilities

Manage and deliver individualized home health care plans
Support patients throughout Western Pennsylvania and nearby areas
Complete charting and documentation accurately and within deadlines
Coordinate scheduling and manage workflow independently
Travel locally between patient locations (approximately 20% of workday)

Schedule

On-call Friday Saturday and Sunday 5:00 p.m. to 10:00 p.m.
8 units on Saturday and 8 units on Sunday
Two holidays per year required

Travel

Local travel required within assigned region

Qualifications
Required

Associate degree or higher in Nursing
At least 1 year of experience in clinical documentation clinical informatics or clinical quality
At least 1 year of experience with EMR systems such as Practice Partner EMR or equivalent
Active Registered Nurse (RN) license in Pennsylvania
Current Advanced Life Support (ALS) and Advanced Cardiac Life Support (ACLS) certifications (American Red Cross or American Heart Association)

Preferred

RN license number provided for credential verification
