Job Summary
We are seeking a dynamic and experienced Physiotherapist or Occupational Therapist to join our Intermediate Care Team working across all pathways including Reablement Rapid Response and Rehabilitation Beds. This is an exciting opportunity to work within a supportive multidisciplinary team (MDT) environment delivering patient-centred care in the community and supporting safe effective transitions of care.
Key Responsibilities
Deliver high-quality evidence-based physiotherapy or occupational therapy interventions across a diverse caseload.
Work collaboratively within a multidisciplinary team to provide holistic care contributing to patient assessment goal setting treatment planning and discharge processes.
Provide therapy input across Intermediate Care pathways:
Reablement supporting patients to regain independence in their own homes
Rapid Response preventing unnecessary hospital admissions
Rehabilitation Beds supporting patients through their recovery in bed-based settings
Conduct community visits to patients homes or care settings delivering timely assessments and interventions.
Liaise effectively with patients families carers and other professionals to ensure continuity of care.
Maintain accurate clinical documentation in line with professional and organisational standards.
Contribute to service development and improvement initiatives as required.
Requirements
Degree or diploma in Physiotherapy or Occupational Therapy.
HCPC registration as a Physiotherapist or Occupational Therapist.
Valid UK driving licence and access to a car for work purposes.
Postgraduate experience within health or social care settings not newly qualified due to the complexity of the caseload.
Experience working as part of a multidisciplinary team (MDT).
Community experience is essential with confidence in lone working and managing time effectively.
Understanding of rehabilitation reablement and/or urgent care pathways.
