Job Summary

3-Month Contract with a Local Authority

Summary
  • This role involves delivering high-quality person-centred social work interventions within a dynamic multi-disciplinary team. The primary objective is to support individuals in crisis aiming to prevent hospital admissions and promote community-based care.
Responsibilities
  • Conduct strengths-based assessments (SBA) and co-create care plans with individuals using a person-centred approach.
  • Address urgent social care referrals safeguarding concerns and crisis situations.
  • Collaborate with a multi-professional team including nurses occupational therapists physiotherapists GPs and support staff.
  • Provide timely social care packages and contribute to medical and nursing interventions.
  • Participate in the Virtual Ward and other referral pathways (e.g. 111 GPs D2A LAS).
  • Complete Continuing Health Care (CHC) assessments for individuals without allocated social care staff.
  • Work one late shift per week (12:30 PM 8:30 PM) and one weekend in four (with Thursday and Friday off that week).
  • Maintain accurate records and contribute to case management and supervision.
  • Engage in ongoing professional development and training provided by the team.
Essential Experience Required
  • Proven experience as a qualified Social Worker in a health or social care setting.
  • Strong understanding of crisis intervention and safeguarding procedures.
  • Experience in multi-disciplinary team working.
  • Desirable: Experience in completing CHC assessments (training will be provided if needed).
  • Ability to work flexibly including evenings and weekends.
Essential Qualification Required
  • Degree in Social Work or equivalent.
  • Registration with Social Work England.
  • Enhanced DBS clearance.
  • Evidence of continued professional development.
Additional Information
  • Working hours: 35 hours per week.
  • The role requires an Enhanced DBS.

JET is a multi- professional team consisting of expert practitioners in Nursing Occupational Therapy Physiotherapists General Practitioners and Social Workers as well as an extensive supporting staff team. The team provide medical and nursing interventions urgent social care packages and safeguarding responses for people over the age 18 years who are registered and live within. These referrals are received via our Virtual Ward 111 GPs D2As and LAS as well as urgent Social Care referrals from the Front Door Service in


Required Education:

Degree in Social Work or with Social Work DBS clearance
