Position Summary
- Provide direct case management services to children and youth meeting annual service delivery unit expectations including treatment service planning assistance in daily living supportive services crisis intervention linkage/referral advocacy and support of clients self-determination.
- Maintain/update necessary client documents records and reports.
- Work collaboratively with other agencies involved in clients care.
- Work collaboratively with multidisciplinary clinical team regarding clients.
- Complete necessary documentation and paperwork according to agency policy.
- Provide transportation of clients in personal vehicle as necessary.
- Work collaboratively with school personnel.
- Go to students home or the community or other setting if necessary and indicated.
- Facilitate referrals and linkages to needed services.
- Provide behaioral health consultatino and education to caregivers/parents
