Position Summary

Provide direct case management services to children and youth meeting annual service delivery unit expectations including treatment service planning assistance in daily living supportive services crisis intervention linkage/referral advocacy and support of clients self-determination.
Maintain/update necessary client documents records and reports.
Work collaboratively with other agencies involved in clients care.
Work collaboratively with multidisciplinary clinical team regarding clients.
Complete necessary documentation and paperwork according to agency policy.
Provide transportation of clients in personal vehicle as necessary.
Work collaboratively with school personnel.
Go to students home or the community or other setting if necessary and indicated.
Facilitate referrals and linkages to needed services.
Provide behaioral health consultatino and education to caregivers/parents

Experience working with children
