Make a lasting difference as a Nurse Practitionerbringing your clinical expertise to enhance resident care and uplift aged care practice.

What youll do

As an experienced Nurse Practitioner youll play a vital role in improving resident outcomes and empowering our clinical teams. Working collaboratively with GPs site leaders and multidisciplinary colleagues youll use your advanced scope of practice to provide timely person-centred care and strengthen clinical capability across Residential Aged Care.

Youll lead by examplementoring staff streamlining communication and applying evidence-based interventions that enhance safety reduce hospital transfers and promote best practice in medication and chronic disease management.

Youll be trusted to make a valuable impact by:

  • Conducting holistic assessments and formulating clinical diagnoses to guide care plans.
  • Partnering with GPs and care teams to ensure effective timely treatment decisions.
  • Leading clinical governance and education to strengthen compliance with Aged Care Standards.
  • Overseeing medication management audits and continuous quality improvement initiatives.
  • Building strong professional relationships that enhance continuity and quality of care.

What youll bring

  • Current Nurse Practitioner registration with AHPRA.
  • Bachelor of Nursing and postgraduate Nurse Practitioner qualifications.
  • Minimum 23 years experience in aged care chronic disease cardiac or infection control.
  • Proven leadership and mentoring ability within a multidisciplinary team.
  • Strong knowledge of Aged Care Quality Standards and clinical governance frameworks.
