We are hiring a Clinical Nurse Specialist Health Failure for our hospital in DublinIreland.
Responsibilities
- Develop proficiency as an expert practitioner in heart failure demonstrating advanced clinical knowledge and specialist skills. Provide evidence-based assessment planning implementation and evaluation of patient care in both inpatient and outpatient settings.
- Offer ongoing support education and advice to patients and their families during and after hospital admission ensuring continuity of care and empowerment through education.
- Act as a nurse prescriber within the Heart Failure Service assist with patient pre-assessments and actively participate in dedicated heart failure clinics and classes as part of a multidisciplinary approach.
- Develop and deliver educational materials resources and programs for patients and staff. Participate in health education initiatives and incorporate preventive strategies into clinical practice.
- Ensure delivery of safe high-quality research-based care. Participate in audits data collection and service evaluation to improve outcomes and maintain compliance with regulatory standards.
- Build lead and support a multidisciplinary team to deliver efficient and effective heart failure services. Promote teamwork communication and accountability across all service areas.
- Contribute to service development through planning organisation and implementation of new initiatives. Support innovation and continuous improvement in heart failure management and patient care delivery.
- Facilitate the professional growth of nursing staff through supervision appraisal and education. Act as a role model demonstrating clinical excellence and commitment to continuous professional development.
- Establish and maintain effective communication with all stakeholders including multidisciplinary teams to coordinate patient care case conferences and clinic scheduling efficiently.
- Use manpower equipment and supplies responsibly and cost-effectively. Support managers in budgetary control and resource optimization while ensuring high standards of patient care are maintained.
Requirements
- Registered on the live register of the Nursing and Midwifery Board of Ireland (NMBI).
- Evidence of ongoing personal and professional development.
- Holds a Higher Diploma in Cardiology or equivalent qualification.
- Minimum of five years post-registration nursing experience.
- Experience in the care and management of patients with heart failure.
- Holds valid certification in Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS).
