Position Summary
The facility is seeking a Board Certified/Board Eligible Palliative Care Physician to join a dedicated and compassionate team in Shelton CT.
This full-time permanent opportunity offers a collegial practice environment with a strong emphasis on patient-centered care symptom management and interdisciplinary collaboration. Physicians will provide expert guidance in managing pain complex symptoms and end-of-life care for patients with serious illnesses.

Essential Position Responsibilities
Conduct comprehensive palliative assessments and develop individualized care plans.
Provide symptom management pain control and supportive care for patients with serious illnesses.
Collaborate closely with primary care providers specialists nurses social workers and chaplains to ensure holistic patient care.
Facilitate discussions with patients and families regarding goals of care advanced directives and end-of-life planning.
Maintain accurate and timely documentation in the electronic medical record.
Participate in team meetings case reviews and quality improvement initiatives.
Promote patient education preventive health strategies and community wellness efforts.
Qualifications
Education: MD or DO degree from an accredited institution.
Licensure & Certification: Board Certified/Board Eligible in Palliative Care or relevant specialty; eligible for Connecticut medical licensure.
Experience: Previous clinical experience preferred but new graduates and residents are welcome to apply.
