We currently have an exciting opportunity for an experienced and committedClinical Manager Home Programs Kidney Care Clinic Transitional Care Unit 21 station incentre dialysis program and Allied Staff to join our Nephrology Program at the Church Site.
In consultation with the Program Director Clinical Practice Leader and Team members the Manager will contribute significantly to the successful development and implementation of the clinical services functional and strategic plan for the specialized unit. If you are looking for an opportunity to lead a teamin one of the largest known Nephrology programs then this is a role for are looking for people who are passionate about promoting and providing the best care to our patients.
As an integral team member of the Nephrology Program there is an opportunity to lead the Program by utilizing a chronic disease prevention and management (CDPM) and self-management approach and philosophy as the Program continues to achieve the deliverables under the Ontario Renal Network and strives to achieve the targets of the Ontario Renal Plan 4:

Hours of Work: Must be available to work Monday to Friday. Occasional weekend work may be required.
