As a Nurse Practitioner with Ovation Hospice you will play a vital role in supporting patients and families through one of lifes most meaningful journeysproviding comfort dignity and expert clinical care at the end of life.

Responsibilities
- Providecomprehensive hospice servicesin compliance with state and federal hospice regulations and thestate Nurse Practice Act
- Perform face-to-face visitsto evaluate hospice eligibility and ensure continued appropriateness for hospice care
- Collaborate with the Interdisciplinary Group (IDG)to develop and update individualized plans of care
- Serve as a medical resourceto clinical and administrative staff providing expertise in pain and symptom management
- Ensure appropriate medical measures are provided tocontrol symptoms and promote patient comfort
- Conduct thorough assessments review clinical information and oversee patient care in coordination with attending physicians and the IDG
- Participate inteam conferences care planning and performance improvement initiatives
- Maintainaccurate and timely clinical documentationin accordance with regulatory requirements
- Demonstrate exceptionalcommunication and counseling skillswith patients families and colleagues
- Stay current onbest practices in hospice palliative care and symptom management
Requirements
- Current and unrestrictedNurse Practitioner licensein the state of Colorado
- Completion of anaccredited Nurse Practitioner program
- Minimum of2 years of hospice palliative care or geriatric experiencepreferred
- Controlled substances registrationwith state and federal authorities (if applicable)
- Strong ability tocommunicate effectively and compassionatelywith patients and families
- Valid drivers licenseand ability to travel throughout the Denver area to patient homes and care settings
- Must not be excluded from participation in theMedicare program
