As the Pharmacist Medication Management at Aledade youll drive meaningful patient outcomes while transforming healthcare delivery. Youll be at the forefront of value-based care partnering with field teams interdisciplinary care members and practices to optimize medication strategies that directly improve patient lives. Youll guide critical transitions of care and elevate quality measures serving as the clinical expert who bridges the gap between complex pharmaceutical needs and real-world care delivery. This role offers the opportunity to shape how Aledades network approaches medication management ensuring every patient receives the right care at the right time.
This Pharmacist will cover our Delaware markets but candidates should be comfortable working remotely/work from home anywhere within the US. However current and valid state pharmacist licensure in the state of Delaware is required.
