Job Summary
Wellness Coordinator Senior Doc
Schedule: Full-time On-site at multiple Senior Living Communities
Pay Range: $23.00 $25.00 per hour (based on experience)
Join Senior Doc a leading provider of mobile and on-site healthcare for senior living communities. Were hiring a Wellness Coordinator to help enhance resident health wellness and overall quality of life through compassionate hands-on support.
In this role youll be the communitys trusted point of contact for care coordination wellness checks and resident engagement. Youll assist with vitals telehealth appointments and communication between residents providers and families ensuring each senior feels supported and connected.
Key Responsibilities
Serve as the Senior Doc representative within the senior living community
Conduct monthly vitals and weights for residents
Support telehealth appointments and assist residents with technology
Perform daily wellness checks and provide companionship
Coordinate communication between residents providers and families
Participate in community wellness and outreach events
Qualifications
High school diploma or GED required
Medical Assistant experience or certification preferred
Prior experience in senior living patient care or care coordination highly valued
