Overview
We are seeking a dedicated and compassionate Registered Nurse (Home Health) to join our clients expanding team in Glenview Illinois. This role focuses on delivering high-quality individualized nursing care to patients in their homes-helping them maintain comfort independence and optimal health. The ideal candidate will be a strong communicator and critical thinker who values both autonomy and collaboration within a supportive team environment.
Key Responsibilities
Conduct comprehensive patient assessments and deliver skilled nursing care in home settings
Develop implement and adjust individualized care plans in coordination with physicians and interdisciplinary team members
Educate patients and caregivers on condition management medications and post-care needs
Evaluate patient progress and ensure continuity of care through regular communication and accurate documentation
Promote patient safety and well-being while adhering to all clinical and regulatory standards
Schedule and Shift Details
Monday through Friday day shift
One weekend per month
Two to three on-call days per month
Two holidays per year
This is a Pay Per Visit position
Travel
Local travel required within the Glenview area (travel reimbursement provided)
Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable transportation and current auto insurance
Preferred
Prior experience in home health or community-based care
Strong organizational clinical assessment and documentation skills
