We are seeking a compassionate and dedicated Registered Nurse (Home Health) to join our clients expanding team in Litchfield Illinois. This rewarding position allows you to provide high-quality individualized nursing care to patients in the comfort of their homes. The ideal candidate will have strong clinical skills excellent communication abilities and a passion for helping patients achieve their health and recovery goals in a home setting.

Key Responsibilities
Conduct comprehensive patient assessments and provide skilled nursing care in the home environment
Develop and implement personalized care plans in collaboration with physicians and interdisciplinary teams
Educate patients and families about medications disease management and post-treatment care
Monitor patient progress document clinical data accurately and adjust care plans as needed
Ensure compliance with all agency policies state and federal regulations
Coordinate with healthcare professionals to ensure seamless and effective care delivery

Schedule and Shift Details
Full-time Monday through Friday
Flexible schedule with no long shifts and minimal weekend rotations throughout the year
This is a Pay Per Visit position

Travel
Travel required within the Litchfield area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health experience preferred)
Current BLS or ACLS certification through the American Heart Association (support provided if needed)
Valid drivers license reliable vehicle and current auto insurance

Preferred
Prior experience in home health hospice or community-based nursing
Strong assessment organizational and communication skills
