We are seeking a skilled and compassionate Registered Nurse (Home Health) to join our clients growing team in Niles this role you will provide high-quality nursing care to patients in their homes ensuring comfort safety and personalized attention. The ideal candidate is detail-oriented empathetic and passionate about delivering exceptional patient care.

Key Responsibilities
Conduct comprehensive patient assessments and provide skilled nursing care in home settings
Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals
Educate patients and families on treatment plans medications and self-care techniques
Monitor and record patient progress reporting any changes to the care team
Maintain compliance with clinical standards policies and documentation requirements

Schedule and Shift Details
Monday through Friday day shift
1 weekend per month 2 3 on-call days per month and 2 holidays per year
This is a Pay Per Visit position

Travel
Local travel within the Niles area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (Home Health experience preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable transportation and active auto insurance

Preferred
Strong clinical assessment and documentation skills
Excellent communication and time management abilities
