Overview
The Speech Therapist provides in-home speech therapy services as prescribed by the attending physician. This position involves assessing planning and implementing rehabilitation programs for speech language and swallowing disorders. The therapist will evaluate patient abilities develop treatment goals document progress and educate families and caregivers to support ongoing recovery. Daily local travel to patient homes is required.
Key Responsibilities
Assess plan and deliver rehabilitation services for speech language and swallowing disorders
Conduct diagnostic evaluations to determine speech and language capabilities establishing goals and expected outcomes
Document patient progress response to treatment and any changes in condition communicating updates to the physician and care team
Educate and guide family members and caregivers on exercises and techniques to improve communication skills
Review and revise treatment plans as needed based on patient progress
Maintain accurate and timely clinical documentation in compliance with organizational and regulatory standards
Participate in team meetings case conferences and professional development activities
Provide referrals to community resources when appropriate
Maintain professional knowledge through continuing education and in-service programs
Perform other related duties as assigned
Schedule and Shift Details
Part-time schedule based on assigned caseload
Monday through Friday flexible daytime hours
Local travel required for home visits; no weekends holidays or on-call

Daily travel required for home visits and team meetings; varied residential environments
Qualifications Required
Masters degree or higher in Speech Language Pathology
Valid Speech Language Pathologist (SLP) license in Illinois (IL)
Current Basic Life Support (BLS) certification (AHA)
Minimum 1 year of supervised post-certification experience in a clinical or institutional setting
Valid drivers license and reliable transportation
Preferred
Experience in home health or community-based care
Familiarity with electronic medical record systems
