Key Responsibilities:
Administer medications and perform wound care
Monitor vital signs and report changes to RN/physician
Assist patients with daily living activities (bathing dressing mobility)
Maintain accurate patient records and clinical documentation
Collaborate with healthcare team for comprehensive care
Educate patients and families on treatment plans and health maintenance
Support a safe clean and organized care environment
Requirements:
Active NY LPN/LVN license
Graduate of accredited program
CPR/BLS certification
Strong communication and teamwork skills
