Job Summary
Job Title: Registered Nurse- Case Manager
Location: Syracuse NY
Type: Contract
Pay rate: $50 - $65/hr
Position Overview:
Navitas Healthcare LLC is seekingRegistered Nurse- Case Managerfor an exciting Travel or Local job in Syracuse NY.
Key Responsibilities:
Perform utilization review and case management for acute care patients.
Ensure compliance with clinical documentation standards.
Collaborate with interdisciplinary teams to coordinate patient care plans.
Use EPIC EMR system for documentation and patient tracking.
Apply evidence-based criteria to support appropriate level of care decisions.
Qualifications:
Current New York State RN License (active and in good standing).
BLS Certification (must be certified or able to be recertified).
Minimum 1.5 years of recent (within 2 years) RN case management experience in an acute care hospital of similar or greater size.
EPIC EMR experience required.
