As a Hospice Admissions RN You will:

  • Visit patients (in homes hospitals nursing facilities etc.) who are referred for hospice care
  • Explain hospice services discuss goals and care with the patient and families answer any questions and obtain necessary consents and legal documents
  • Obtain medical history current medications and symptoms
  • Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
  • Coordinate the start of services (nursing aides equipment medications.)
  • Collaborate with interdisciplinary team
  • Maintain accurate and timely documentation

Qualifications

  • Graduate of an accredited nursing school with current licensure to practice in state of operation
  • One year of hospice nursing experience 3 years preferred or similar setting such as Long-Term Care Intensive care or Emergency room will be considered in place of hospice nursing experience
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
