Overview
We are seeking a compassionate and dedicated Registered Nurse (Home Health) to join our clients expanding team in Antioch this role you will provide skilled nursing care to patients in their homes supporting recovery comfort and independence. This position is ideal for an experienced RN who values flexibility autonomy and delivering personalized patient care in a home setting.
Key Responsibilities
Deliver skilled nursing care to patients in their homes according to the physicians plan of care
Conduct comprehensive patient assessments and develop individualized care plans
Monitor and evaluate patient progress and adjust care plans as needed
Provide education to patients and caregivers regarding disease management medications and home safety
Document all care and communication accurately and timely in compliance with regulations
Collaborate with interdisciplinary team members to coordinate and optimize care
Participate in regular case conferences and team meetings
Schedule and Shift Details
Monday Friday daytime schedule
Includes one weekend per month 2 3 on-call days per month and two holidays per year
This is a Pay Per Visit position
Travel
Local travel to patient homes required within the Antioch area
Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health experience preferred)
Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification through the American Heart Association
Valid drivers license reliable transportation and auto insurance
Preferred
Experience in home health or community-based nursing
Strong clinical documentation and time management skills
