Overview
We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our clients growing team in Berwyn Illinois. This role focuses on providing high-quality nursing care to patients in the comfort of their homes supporting their independence recovery and overall well-being. The ideal candidate will demonstrate strong clinical judgment attention to detail and a passion for patient-centered care.
Key Responsibilities
Deliver skilled nursing care to patients in their homes according to individualized care plans
Conduct comprehensive patient assessments and evaluate ongoing progress
Educate patients and caregivers on medication management disease prevention and home safety practices
Document all nursing care accurately and in compliance with clinical and regulatory standards
Communicate effectively with physicians caregivers and interdisciplinary team members
Participate in team meetings case conferences and continuous professional development activities
Schedule and Shift Details
Monday Friday daytime schedule
Includes one weekend per month 2 3 on-call days per month and two holidays per year
This is a Pay Per Visit position
Travel
Local travel required within the Berwyn area
Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
At least 1 year of professional RN experience (home health preferred)
Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification through the American Heart Association
Valid drivers license reliable transportation and auto insurance
Preferred
Previous experience in home health or community-based nursing
Excellent organizational documentation and time management skills
