We are seeking an experienced and compassionate Registered Nurse (Home Health) to join our clients team in Carlinville Illinois. This position focuses on delivering personalized nursing care to patients in their homes helping them achieve optimal health outcomes while maintaining comfort and independence. The ideal candidate will be self-motivated detail-oriented and committed to providing exceptional patient-centered care.

Key Responsibilities
Provide skilled nursing care in patients homes based on individualized care plans
Conduct comprehensive patient assessments and evaluate ongoing needs and progress
Educate patients and caregivers on disease management medications and home safety practices
Maintain accurate and timely clinical documentation in compliance with healthcare regulations
Communicate effectively with interdisciplinary team members to ensure coordinated care
Participate in team meetings case conferences and training opportunities to support continuous professional development

Schedule and Shift Details
Monday Friday daytime hours
Includes one weekend per month 2 3 on-call days per month and two holidays per year
This is a Pay Per Visit position

Travel
Local travel required within the Carlinville area

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health preferred)
Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification through the American Heart Association
Valid drivers license reliable transportation and auto insurance

Preferred
Previous experience in home health or community-based nursing
Strong clinical assessment communication and time management skills
