Overview
We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our clients growing team in Grantville Pennsylvania. This role focuses on providing exceptional patient-centered nursing care within the comfort of patients homes. The ideal candidate will demonstrate strong clinical judgment excellent communication and a passion for helping patients achieve optimal health outcomes in a supportive home environment.

Key Responsibilities
Provide skilled nursing care and conduct comprehensive assessments for patients in their homes
Develop and implement individualized care plans in collaboration with physicians and the healthcare team
Educate patients and families on health management medications and treatment plans
Monitor patient progress and adjust care plans as needed to ensure positive outcomes
Maintain accurate timely and compliant documentation in accordance with agency and regulatory standards
Coordinate with interdisciplinary team members to deliver holistic quality care

Schedule and Shift Details
Monday through Friday 8:00am 5:00pm
On-call weekend rotation every 4 8 weeks (Saturday & Sunday 8:00am 5:00pm)
Flexible scheduling and limited weekend work
This is a Pay Per Visit position

Travel
Local travel within the Grantville area required (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association (training assistance available if needed)
Valid drivers license reliable vehicle and current auto insurance

Preferred
Prior experience in home health or community-based nursing
Strong assessment organizational and interpersonal skills
