We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our clients growing team in Greentown Pennsylvania. Key Responsibilities
Provide skilled nursing assessments treatments and interventions to patients in their homes
Develop and implement personalized care plans in coordination with the healthcare team
Educate patients and caregivers on disease management medications and post-care instructions
Monitor patient progress and adjust treatment plans as necessary
Maintain accurate and timely clinical documentation per regulatory standards
Collaborate with multidisciplinary team members to ensure comprehensive coordinated care

Schedule and Shift Details
Monday through Friday 8:00am 5:00pm
On-call weekend rotation every 4 8 weeks (Saturday and Sunday 8:00am 5:00pm)
Flexible scheduling with minimal weekend work
This is a Pay Per Visit position

Travel
Local travel required within the Greentown area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association (training assistance available if needed)
Valid drivers license reliable vehicle and current auto insurance

Preferred
Previous home health or community nursing experience
Excellent clinical judgment communication and organizational skills
