Overview
We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our clients expanding team in Lebanon Pennsylvania. This position provides an opportunity to deliver personalized high-quality nursing care to patients in their homes-supporting recovery independence and overall wellness. Key Responsibilities
Provide skilled nursing care and perform comprehensive patient assessments in the home setting
Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams
Administer medications treatments and necessary clinical interventions
Educate patients and caregivers on disease management medications and follow-up care
Monitor progress adjust care plans as needed and document all care accurately and promptly
Communicate effectively with the care team to ensure coordinated and consistent patient care

Schedule and Shift Details
Monday through Friday 8:00am 5:00pm
On-call weekend rotation every 4 8 weeks (Saturday and Sunday 8:00am 5:00pm for admissions as needed)
Two weekdays off following on-call weekends
Flexible scheduling with minimal weekend requirements
This is a Pay Per Visit position

Travel
Local travel required within the Lebanon PA area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association (support provided if needed)
Valid drivers license reliable transportation and current auto insurance

Preferred
Previous experience in home health or community-based care
Strong communication clinical assessment and organizational skills
