We are seeking a dedicated and compassionate Registered Nurse (Home Health) to join our clients expanding team in Marysville Pennsylvania. This position focuses on delivering skilled patient-centered care directly in patients homes-promoting independence recovery and overall well-being. The ideal candidate will bring clinical expertise excellent communication skills and a strong commitment to providing exceptional care.

Key Responsibilities
Deliver professional nursing care and perform comprehensive patient assessments in home settings
Develop and manage individualized care plans in coordination with physicians and interdisciplinary teams
Educate patients and families on care plans medications and lifestyle adjustments
Monitor patient progress and modify care plans as necessary to ensure optimal outcomes
Maintain accurate and compliant documentation of all care provided
Communicate effectively with care team members to ensure seamless coordination

Schedule and Shift Details
Monday through Friday 8:00am 5:00pm
Flexible scheduling with minimal weekend work
On-call weekend rotation every 4 8 weeks as needed
This is a Pay Per Visit position

Travel
Local travel required within the Marysville area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association (assistance available if not current)
Valid drivers license reliable transportation and current auto insurance

Preferred
Previous experience in home health or community-based nursing
Strong time management communication and assessment skills
