Overview
We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our clients expanding team in Shenandoah Pennsylvania. This position focuses on providing high-quality individualized nursing care to patients in their homes-supporting their health independence and comfort. The ideal candidate will demonstrate strong clinical judgment adaptability and a genuine passion for patient care.

Key Responsibilities
Perform comprehensive patient assessments and deliver skilled nursing care in home settings
Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary care team
Educate patients and families on disease processes medications and post-care instructions
Monitor patient progress evaluate outcomes and adjust care plans as needed
Ensure accurate and timely clinical documentation that complies with all standards and regulations
Coordinate with healthcare providers and team members to promote effective continuous care

Schedule and Shift Details
Monday through Friday 8:00am 5:00pm
On-call weekend rotation every 4 8 weeks (Saturday and Sunday 8:00am 5:00pm)
Two weekday compensatory days off following on-call weekends
Flexible scheduling with minimal weekend work
This is a Pay Per Visit position

Travel
Local travel required within the Shenandoah area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association (support provided if needed)
Valid drivers license reliable vehicle and current auto insurance
Ability to work independently while managing multiple priorities
Strong clinical assessment communication and documentation skills

Preferred
Prior experience in home health or community-based care
Knowledge of OASIS documentation standards and home health regulatory compliance
