Schedule: 40 hours per week 11am - 7:30pm Monday - Friday
Department: Home Care PACE of Southwest Michigan St. Joseph MI
What Youll Do in This Role:
The Certified Nursing Assistant (CNA) provides essential in-home care and assistance to participants enrolled in the PACE program. You will work closely with the Home Care Supervisor to deliver compassionate person-centered care and support activities of daily living.
Primary Responsibilities:
- Provide assistance support and guidance for participants in their homes
- Travel to participant residences to provide personal care
- Assist with bathing dressing toileting transfers and mobility
- Use mechanical lifts for safe transfers when required
- Record participant weights and report changes to supervision
- Support housekeeping tasks such as laundry sweeping and grocery shopping
- Assist participants on/off transportation as needed
- Maintain open communication with the care team about changes in participant condition
- Help uphold a safe and clean environment
- Participate in Quality Improvement activities
- Perform all other duties as assigned
What Youll Need:
- High School Diploma or equivalent
- Certified Nursing Assistant (CNA) certification in the State of Michigan
- Valid Michigan Drivers License with a clean driving record
- Current CPR certification or willing to obtain.
- Minimum of 1 year experience as a personal care aide CNA or EMT
- Ability to work independently multitask and operate as part of an interdisciplinary team
- Willingness to perform housekeeping and personal care duties
- Experience working with older adults is highly preferred
