Overview
We are seeking a dedicated and compassionate After Hours Hospice Registered Nurse (RN) to join our hospice care team in the Glen Carbon and Springfield Illinois areas. The ideal candidate is an experienced hospice nurse who can manage after-hours patient needs provide compassionate in-home care and work independently while coordinating with the larger hospice team.
Key Responsibilities

Deliver and manage individualized hospice patient care plans

Provide after-hours and on-call nursing visits as needed within the service area

Support patients and families with symptom management comfort measures and emotional guidance

Collaborate with the hospice care team to ensure continuity and quality of care

Maintain timely and accurate documentation using EMR systems

Manage after-hours call schedules prioritize urgent visits and meet required charting deadlines

Utilize tablets mobile devices and EMR software for communication and documentation
Schedule and Shift Details

Schedule: Seven consecutive evenings Monday through Sunday 5:00 p.m. 8:00 a.m.

After completing the seven-night rotation you will have the following seven days off
Travel

Local travel required within the Glen Carbon and Springfield service areas
Qualifications
Required
-
Associate degree in Nursing or higher

Current Registered Nurse (RN) license in the state of Illinois

Minimum of 2 years of hospice nursing experience

At least 1 year of experience using electronic medical record (EMR) systems

Valid Drivers License reliable transportation and active auto insurance
Preferred

Experience with TruChart EHR or similar EMR platforms

Strong background in hospice palliative or home health nursing
