Key Responsibilities

Provide management and delivery of individualized patient care plans
Collaborate with interdisciplinary team members to ensure quality outcomes
Support patients in Irwin Monroeville Murrysville and nearby communities
Meet critical documentation and charting deadlines
Effectively manage time and scheduling responsibilities
Utilize EMR software mobile devices and clinical technology in daily workflow

Schedule and Shift Details

Full-time 40 hours per week
Evening and weekend shifts required

Travel

Occasional local travel within assigned service areas

Qualifications

Required

Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of nursing experience
Experience with Electronic Medical Record (EMR) systems such as TruChart EHR or similar
Valid drivers license and reliable transportation
Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification from the American Heart Association
Willingness to travel locally as needed

Preferred

Previous experience in home health or community-based nursing
RN license number provided upon application
