Overview
Join our client s exceptional Home Health team as a Physical this role you ll provide high-quality compassionate care to patients in their homes helping them regain independence and improve their quality of life. This full-time position offers flexibility professional growth and a supportive team-based environment.
Key Responsibilities
Conduct comprehensive physical therapy assessments and develop individualized care plans
Implement therapeutic interventions to improve mobility and function
Educate patients and caregivers on exercises equipment use and safe practices
Supervise physical therapy assistants and other support staff as needed
Maintain accurate and timely documentation within EMR systems
Collaborate with interdisciplinary care teams to ensure coordinated patient care
Schedule and Shift Details
Monday Friday 8:00 AM 5:00 PM
One weekend per month
Two holidays per year (rotating schedule)
No on-call responsibilities
Travel
Local travel to patients homes within assigned territory
Qualifications Required
Bachelor s degree or higher in Physical Therapy from an accredited institution
Valid Physical Therapist license in Pennsylvania (PA)
Minimum of 1 year of experience in a healthcare setting
Experience using Electronic Medical Record (EMR) systems
Valid driver s license and reliable transportation
Preferred
