Key Responsibilities
Provide direct patient care in the home setting in accordance with physician orders and individualized care plans
Conduct comprehensive assessments monitor patient progress and document care accurately and promptly
Educate patients and families on disease management medication administration and home safety
Collaborate with physicians case managers and the interdisciplinary team to coordinate ongoing care
Ensure compliance with all regulatory standards and company policies
Maintain professional communication and deliver compassionate patient-centered service
Schedule and Travel
Full-time position Monday through Friday
Flexible scheduling with minimal weekend rotation
Travel required within the local Western Pennsylvania region (mileage reimbursement provided)
Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of professional RN experience (Home Health experience preferred)
Valid driver s license reliable transportation and current auto insurance
Current certification in one of the following:
American Heart Association Basic Life Support (AHA BLS)
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Preferred
Previous home health experience
Excellent time management and communication skills
