Job Summary:
The Registered Nurse will be responsible for providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and other members of the team is critical.

Responsibilities:
- Provide skilled nursing care to patients with various medical conditions.
- Conduct patient assessments and develop individualized treatment plans.
- Administer medications and treatments as prescribed by physicians.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Document patient care accurately and timely.
- Participate in case management and discharge planning.
- Provide education and support to patients and their families.
Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
- Acute care experience
Preferred Skills & Certifications:
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room
Special Considerations:
- Floating Expectations within the Laguna Facility
- All RTO must be submitted upfront
