Job Summary:
The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. This role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. The position requires strong communication and collaboration skills with patients providers and team members.

Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- BLS certification
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room
Special Considerations:
- Mandatory float to other facilities as needed
- No holiday RTO is accommodated
- No RTO the day before or after a holiday
Scheduling:
- Full-time
- 5 shifts per week
- Every other weekend
- No block scheduling accommodated
