Job Summary:

The Registered Nurse will deliver skilled nursing care case management and hospice support to individuals dealing with age-related issues frailty and serious illnesses. Key responsibilities include patient assessments medication administration treatment planning and teamwork with an interdisciplinary group. Effective communication and collaboration with patients providers and team members are essential. A minimum of 2 years of experience with exceptional skill is required.
