The Region 4 Care Navigator is responsible for providing follow-up care coordination and service navigation for individuals referred through the PRS/Hopelink Crisis Call Center. This position plays a key role in supporting timely access to behavioral health and community services by collaborating with local Community Services Boards (CSBs) providers and support systems. The Care Navigator works closely with individuals experiencing behavioral health crises to ensure connection to appropriate services and supports reduce barriers and promote recovery and wellness. Work is performed under general supervision and requires independent judgment knowledge of community-based services and the ability to work collaboratively within a regional system of care. Essential Functions - Provide follow-up contact to individuals referred through the PRS/Hopelink Crisis Call Center to ensure connection to recommended services and supports.
- Coordinate with CSBs and other community providers to support same-day access to services.
- Assist individuals seeking to re-engage with behavioral health services following a crisis event.
- Monitor referrals and ensure that appropriate follow-up actions are taken based on individual needs.
- Document all service coordination efforts in accordance with program regulatory and contractual requirements.
- Support ongoing collaboration between the Call Center CSBs regional partners and stakeholders to promote continuity of care.
- Participate in case review meetings regional planning and quality improvement initiatives as assigned.
- Address barriers to care including transportation scheduling and linkage to housing or peer support services.
- Maintain knowledge of available resources across Region 4 and assist in resource development and updates.
- Participate in special projects workgroups and committees as assigned.
- Support client-centered trauma-informed and recovery-oriented practices in all interactions.
- Perform other duties as assigned.
- Assist in the development and distribution of regional service guides and resource directories.
- Facilitate outreach and engagement efforts for individuals who are difficult to reach or at risk of disengagement.
- Provide brief psychoeducation or orientation to available services to support informed decision-making.
- Represent the Region 4 Crisis Hub in community events resource fairs and stakeholder meetings as training or mentorship to new navigation staff interns or community partners as requested.
- Support crisis system performance monitoring through data collection and reporting assistance.
