Job Summary
Job Title: Case Manager Registered Nurse
Location: Syracuse NY 13210
Shift: Day Shift 8:00 AM 5:00 PM (Monday Friday) 40 Hours per Week
Pay Rate: $65 per hour
Position Overview
We are seeking an experienced Registered Nurse (RN) Case Manager to join a dynamic healthcare team in an acute care setting. The ideal candidate will provide patient-centered care coordination ensuring safe efficient and effective transitions throughout the continuum of care. This role focuses on collaboration advocacy and the efficient utilization of resources to achieve optimal patient outcomes.
Key Responsibilities
Conduct comprehensive assessments of patient needs and develop individualized care plans.
Coordinate and monitor transitions of care including admission transfer and discharge processes.
Serve as a liaison among interdisciplinary team membersphysicians nurses social workers payers and external agencies.
Facilitate utilization review and ensure alignment with payer requirements and medical necessity standards.
Educate patients and families regarding discharge planning available community resources and follow-up care.
Accurately document all case management activities in the electronic health record (EHR) system.
Ensure compliance with regulatory accreditation and payer standards.
Participate in performance improvement initiatives and departmental quality projects.
Required Qualifications
Active New York State RN license (required).
Basic Life Support (BLS) certification with ability to maintain current status.
Minimum 1.5 years of recent experience (within the past 2 years) in acute care hospital case management at a facility of similar size and complexity.
Proficiency in Epic (EHR system) is required.
Demonstrated ability to apply critical thinking effective communication and strong organizational skills.
Proven ability to work collaboratively in a multidisciplinary fast-paced environment.
