TITLE: Help Me Grow Home Visitor
SUPERVISOR: Home Visiting Contract Manager & Supervisor Help Me Grow
FUNCTION: Early Childhood/Family-Community Partnerships
JOB LOCATION: Stark County Educational Service Center
BEGINNING SALARY:$41096
JOB OBJECTIVE: To work in partnership with families to promote healthy pregnancies improve child health/development/school readiness improve parent confidence and competence and increase family connectedness to community and social supports. Deliver the following components to all families: an evidence-based parenting education curriculum ongoing screenings and assessments of the child(ren) and family family-specific need-based referrals and resource linkages and transitioning of child(ren) to a development-enhancing program/early care and education provider by age three.
JOB REQUIREMENTS:
- Minimum of an Associates Degree in a field of study relating to child and family studies child life early childhood education social services child development and nursing/other applicable healthcare field.
- At least two years experience working with families and children prenatal to age three including the ability to establish trusting relationships.
- Knowledge of infant and child development and experience using child development assessment tools.
- Knowledge of community resources.
- Home visiting experience is preferred.
- Open to reflective practice (i.e. has capacity for introspection communicates awareness of self in relation to others and recognizes value of supervision.
- Data entry experience.
- Valid State of Ohio drivers license and proof of insurance.
- Acceptance of individual differences.
- Experience and willingness to work with the culturally diverse populations within Stark County.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
- Determine familys eligibility to receive Help Me Grow Home Visiting services.
- Assure that every family has a completed family-directed plan with at least one family goal as it relates to any of the four goals of the Help Me Grow Home Visiting program within mandated timelines.
- Assure that parents have been informed of their rights.
- Assure that developmental screenings of the child(ren) have been completed. If a delay is suspected then a referral for a developmental evaluation must be made with the consent of the parent.
- If appropriate referral and coordination of developmental evaluation and ongoing assessment in all five developmental domains the family assessment and hearing/vision/nutrition screenings to determine dual eligibility/re-determination for Part C services.
- Complete required tools used for parent/home assessment.
- Facilitate and participate in the development implementation review and monitoring of the Family Plan and their timelines.
- Facilitate development of family outcomes.
- Parent education using the Growing Great Kids curriculum.
- Assist parents in identifying establishing and communicating with a medical/health home.
- Identify and connect families to specific need-based resources and providers.
- Inform families of the availability of advocacy services.
- Coordinate with medical and health providers.
- Coordinate transition of child(ren) to a development-enhancing program/early care and education provider by age three.
- Responsible for maintaining appropriate documentation for the client record (hard copy and electronic data systems) according to ODH and local rules/policies. Using this documentation will work with Home Visiting Supervisor to assure accurate and timely Medicaid billing.
- Communicates and cooperates effectively professionally and respectfully with families/schools/professionals and all others involved in the Help Me Grow system.
- Contributes to team effort by integrating individual responsibility with other team member responsibilities.
- Demonstrates commitment to continuous learning and self-improvement by reading and attending such meetings conventions and conferences as are necessary to keep abreast of the latest Help Me Grow developments and trends.

- Maintains a consistent pattern of positive work habits and personal traits:
- Punctuality promptness accuracy and thoroughness.
- Adaptability dependability and initiative.
- Loyalty courtesy and professionalism in appearance and public relations activities.
- Establishing a positive rapport with families children colleagues and all other staff.
- Ensuring strict confidentiality regarding matters involving children families and staff.
- Complies with the policies and regulations of the county Superintendent of Schools and the mandated state Help Me Grow Rules.
- Performs such other duties as may be assigned by the Home Visiting Contract Manager & Supervisor and/or the county superintendent of schools or his/her designee.
- Obtains and maintains State of Ohio Home Visitor credentials according to state Help Me Grow mandate.
- Evening and weekend hours may be required.
