22.36 - 26.83 USD Per Hour

Working Location:

San Leandro: San Leandro Center (In-Person Position)

Working Hours/Shift:

Monday-Friday AM Shift

What you bring to SBHG (qualifications):

  • 4 years of experience in a social service field OR
  • Associate Degree plus 2 years of experience in a social service field OR
  • Bachelors degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service
  • Valid California Drivers License

NOTE: Must possess a valid California Drivers Licenseand maintain a driving record that meets the company drivers eligibility policy.

How you will make a difference (job overview):

This position has primary responsibility for case management services to clients and their families referred through Stars Transition Age Youth (TAY) Program (TAY)

Division/Program Overview:

  • Outpatient community-based program that serves youth age 18-25 with serious and persistent mental health issues
  • Program provides intensive case management outreach and engagement medication support as well as mental health counseling peer groups and supportive services to assist TAY transition to adult life.
  • Services offered are trauma informed and culturally responsive

In accordance with California law the grade for this position is 22.36 - 33.54. Placement within the grade is determined based on experience internal equity and other factors permitted by law.
