Responsibilities:

Monitor and accurately record patients vital signs
Ensure adherence to medication schedules and provide guidance to patients

Conduct follow-up visits to support ongoing home health care needs

Qualifications:

Active LVN license in Oregon
Home health experience preferred
Valid drivers license with a clean driving record
