Job Summary
Home Health Physical Therapist
Requirements:
o Must have State Physical Therapist License
o Must meet productivity expectation of 30 points per week
o 1 year of home health experience preferred in the last 3 years
o HCHB experience preferred
Location: Chillicothe OH
Pay Rate: $55/hr & $.50/mile
Shift Details: Monday-Friday 8hr shifts 40 hrs a week no weekends/holidays
Job Description:
o Patient Assessment: Conducts thorough evaluations to determine the patients needs and goals.
o Treatment Plan Development: Creates individualized therapy plans often in consultation with physicians.
o Treatment Implementation: Provides hands-on therapy exercises and education to patients.
o Documentation: Maintains accurate records of patient progress and interventions.
o Communication: Communicates with physicians and other healthcare team members to ensure coordinated care.
o Patient and Caregiver Education: Provides education and counseling to patients and their families about their condition and treatment.
o Supervision: May supervise physical therapist assistants in accordance with applicable regulations according to Indeed.
o Progress Monitoring: Regularly evaluates patient progress and adjusts treatment plans as needed
Additional information:
o 13 week contract with possible extensions
