Schedule: Flexible 8-hour shifts MondayFriday (No weekends)
Options: 7:00 AM3:00 PM 8:00 AM4:00 PM or 8:00 AM5:00 PM
Provide specialized wound ostomy and continence care for residents in a skilled nursing or long-term care setting. Serve as a clinical resource for staff ensuring residents receive effective care and education.
Perform wound assessments reassessments and management per policies and procedures.
Develop skin care programs to prevent breakdown.
Implement and revise care plans for residents with wounds fistulae ostomies or incontinence.
Assess and manage pain; educate residents and families on pain management.
Serve as a resource for nursing staff providing in-services on wound prevention and care.
Communicate clearly with nursing staff and physicians; consult with healthcare professionals for interdisciplinary care.
Ensure documentation meets current standards; attend continuing education to stay current in wound care practices.
Active RN or LPN license in Arizona
Certification as a Wound Ostomy and Continence Nurse (CWOCN) or Certified Wound Care Nurse (CWCN)
Arizona Fingerprint Clearance Card (or ability to obtain)
Current CPR/BLS certification
Experience in a skilled nursing facility or long-term care
