Job Description
In this role you will be responsible for evaluating patients nutritional status as well as plan coordinate and manage the nutritional care of our patients. You will also build relationships with physicians and nursing to achieve a stellar standard of care to ensure patients nutritional needs and goals are met.
Responsibilities
- Conduct comprehensive nutritional assessment for patients by collecting recording and interpreting data from patients family members caregiver and other individuals
- Formulate nutrition intervention plan based on nutrition related concerns
- Work together with health care providers patient and caregiver to implement and communicate the nutrition care plan and expected outcomes
- Provide nutritional counseling and education to patient families and caregivers
- Maintain accurate record keeping of daily clinical activities
Qualifications
- Bachelors Degree in Dietetics Nutrition or related field
- Registered Dietitian credentials
- Two years of clinical/ practical experience. Geriatrics experience preferred
- Excellent verbal and written and oral communication skills.
